Job Summary

ABOUT THE ROLE


Our client, a well-established UK home care provider, is looking for an experienced Care Planning Officer to join their clinical team on a part-time basis.


This role is ideal for someone with a nursing or clinical background who has hands-on experience writing care plans and reviewing care documentation. You will play a key role in ensuring care plans, risk assessments and medication records remain accurate, compliant and reflective of each client's current needs.


Working closely with the Clinical Director, you will help maintain exceptional standards of care, quality assurance and regulatory compliance across the organisation.


KEY RESPONSIBILITIES


- Review daily care records and identify changes in client needs, deterioration and emerging risks.

- Review and update care plans in line with current client needs and UK CQC standards.

- Review and maintain risk assessments to ensure they remain accurate and up to date.

- Review and update medication records (eMAR) following confirmed medication changes.

- Produce reports highlighting concerns, risks and trends for the clinical team.

- Liaise with carers and healthcare professionals to verify information and maintain accurate documentation.

- Monitor the quality and completeness of care records.

- Support internal quality assurance, governance and audit processes.

- Ensure all documentation remains accurate, compliant and inspection ready.


REQUIREMENTS


- Registered Nurse or significant clinical experience within health or social care.

- Previous experience writing and reviewing care plans.

- Strong understanding of UK CQC care planning standards.

- Experience reviewing risk assessments.

- Excellent written and verbal English communication skills.

- Exceptional attention to detail.

- Strong IT skills and confidence using digital systems.

- Comfortable working independently in a remote environment.


NICE TO HAVE


- Previous experience within domiciliary care or live-in care.

- Experience using Log My Care or similar care management software.

- Experience reviewing medication records (eMAR).

- Previous quality assurance or governance experience.
