Job Summary
Assess the resident’s physical, psychological & social needs & compile a care plan in collaboration with the resident, family & any other relevant health care professional.
Plan, implement & evaluate the nursing care in relation to the residents needs in accordance with clinical policy & regulatory framework/s with other appropriate health care professionals.
Ensure all entries into the nursing records meet NMC standards for record keeping.
To actively participate in quality initiatives and to ensure that National Minimum Care Standards are achieved and maintained.
Act as keyworker for nominated residents by planning, assessing and evaluating Care Plans and Risk Assessment and carrying out weekly key worker sessions.
Follow up to ensure care plans are being carried out day to day.
Monitor and manage the changing needs of residents and to bring these to the attention of staff team, Home Manager and the multi-disciplinary team.
Completing DoLS applications and reviews.
Respect residents’ autonomy, dignity and confidentiality.
Encourage and support residents in reaching their optimum levels of achievement and independence.
Maximise the choices available to resident.
Ensure residents’ files are kept up to date, person centred and to a professional standard.
Prepare for, and participate in staff, nurse, residents’, MDT, clinical governance meetings, recording and storing accurate minutes/documentation. Following up on actions identified.
To undertake all mandatory training courses and participate in CPD.
Ensure a good standard and knowledge of medication management and compliance, ordering, storage, administration and auditing of medication.
Competently complete clinical nursing procedures e.g. administration of depo, taking BMs etc.
Support the Manager in carrying out all necessary audits to ensure compliance e.g. infection control, medication fridge/clinic room temperature.
Assist the Registered Manager in implementing values, policies, procedures and systems for maintaining the highest standard of care.
Effectively communicate with residents, colleagues and multi-disciplinary team to strive for the maximum effectiveness of everyday running of the home and best quality care for residents.
At all times be a professional and respectable role model.
Provide supervision for junior staff and ensure they adhere to policies, procedures and systems in place within the company.
Under the guidance and supervision of the Registered Manager, exercise leadership within the service through effective communication, organisation and client and employee relationships.
Ensure compliance with the requirements of all legal and governing bodies including the requirements of the Care Quality Commission and work in accordance with all relevant legislation including requirements of NMC.
Keep up dated on new clinical and nursing developments.
