The service provides specialist 24-hour nursing care for individuals living with advanced dementia and other complex needs that may not always be fully supported within traditional care environments.

We are looking for a skilled and compassionate Registered Nurse who shares our commitment to providing outstanding care. You will be confident delivering high standards of clinical practice, working collaboratively with residents, families and multidisciplinary teams, and supporting a culture where every resident is treated with dignity, respect and individuality.You will have experience within dementia care and a passion for supporting people with complex needs. You will be committed to delivering safe, effective and person-centred care while contributing positively to the wider care team.

Key Responsibilities

As a Registered Nurse, you will:Assess residents’ physical, psychological and social needs and develop personalised care plans in partnership with residents, families and relevant healthcare professionals.

Plan, implement and evaluate nursing care in line with clinical policies, regulatory frameworks and best practice guidance.

Ensure all nursing records are completed accurately and meet NMC standards for record keeping.

Act as a keyworker for allocated residents, reviewing care plans, completing risk assessments and holding regular keyworker sessions.

Ensure care plans are implemented effectively and reflect residents’ changing needs.

Monitor residents’ health and wellbeing, identifying changes and communicating these to the care team, Registered Manager and wider multidisciplinary team.

Complete and review Deprivation of Liberty Safeguards (DoLS) applications.

Respect and promote residents’ dignity, privacy, confidentiality, independence and personal choices.

Ensure residents’ records are maintained to a high professional standard and reflect individual needs and preferences.

Participate in staff meetings, resident reviews, multidisciplinary team meetings and clinical governance meetings, ensuring actions are documented and followed up.

Maintain professional development through mandatory training and continuing professional development (CPD).

Ensure safe medication management including ordering, storage, administration, auditing and compliance with relevant guidance.

Competently undertake clinical nursing procedures including blood monitoring and other relevant interventions.

Support the Registered Manager with clinical audits including infection prevention and control, medication management and equipment checks.

Promote compliance with CQC standards, NMC requirements and all relevant legislation.

Provide supervision, guidance and support to junior members of staff.

Act as a professional role model, demonstrating excellent communication, leadership and clinical standards.

Keep up to date with developments in nursing practice, dementia care and clinical guidance.

Skills and Experience Required

Essential:

Registered Nurse qualification with current NMC registration.

Experience working within dementia care.

Understanding of person-centred care approaches.

Knowledge of regulatory requirements and clinical frameworks.

Understanding of Mental Capacity Act (MCA), DoLS and safeguarding requirements.

Knowledge and experience of safe medication management.

Evidence of continued professional development.

Strong communication and teamwork skills.

Ability to demonstrate leadership and support colleagues effectively.

A positive, compassionate and caring approach.

Commitment to delivering high quality, values-led care.

Desirable:

Experience working within a specialist dementia or complex care environment.

Experience supporting individuals with advanced dementia and complex needs.

Experience contributing to audits, quality improvement and clinical governance.
