Job Summary
Our client supports children and adults with a range of complex care and support needs including learning disabilities, autism, mental health, behaviours that challenge and chronic conditions. They enable each person to live as independently as possible in their own home and community. Each package of care and support is tailored to meet the person’s individual needs and requirements, goals and aspirations. Experience
Level 3 in Health and Social Care Full UK driving licence and your own car Demonstrable caring nature and at least one years’ experience in a care role Experience dealing with or within a high-pressure environment Experience using apps for electronic care note taking Clear and concise written and spoken English communication skills Learning disabilities and mental health Empathy and independence building skills for supported individuals Strong time management and organisational skills Demonstrable problem-solving skills Ability to manage multiple tasks and changing priorities High attention to detail Working with people who may challenge / tendency towards aggression Knowledge and understanding of the healthcare sector and relevant legislation the social care sector, commissioning, and local authorities Personal Attributes Career focussed and eager to learn Proactive and organised. Job Responsibilities
- Best Practice Care: Provide care in line with best practices and legislative requirements, adhering to policies and procedures under the guidance of senior staff.
- Person-Centred Support: Deliver individualised, person-centred care that meets the unique needs of each supported individual.
- Care Plan Collaboration: Assist senior carers and registered managers in proactively updating care plans, elevating issues as needed.
- Community Engagement: Support individuals in building and maintaining relationships within their local community.
- Safeguarding: Promote and safeguard the welfare of individuals receiving support.
- Documentation Compliance: Ensure adherence to care plans and maintain relevant paperwork.
- Electronic Records: Use electronic care notes efficiently, with training provided.
- Multi-Disciplinary Teamwork: Collaborate with other members of the multi-disciplinary team to provide comprehensive care.
