We are seeking a compassionate and dedicated Physical Therapist to join our exceptional home health care team. In this role, you will provide personalized physical therapy services to patients in their homes, collaborating with a team of healthcare professionals to manage and deliver patient care plans. Key Responsibilities:
Patient Care: Provide physical therapy services to patients in their home settings, developing and implementing individualized care plans.
Collaboration: Work with a multidisciplinary care team to ensure effective patient management and care coordination.
Travel: Spend approximately 20% of your workday traveling to and from patient homes within the Dingmans Ferry area and surrounding locations.
Documentation: Consistently meet deadlines for charting, utilizing state-of-the-art technology to streamline documentation and reduce time spent on administrative tasks.
Mentorship and Development: Engage with clinical educators, preceptors, and supervisors for guidance and professional growth.
Flexibility: Manage your own schedule with support from dedicated schedulers, ensuring a balance between work and personal commitments.
Schedule and Shift Details:
Full-time position: Monday to Friday, 8:00 AM – 5:00 PM.
1 weekend per month and 2 holidays per year required.
No on-call duties.
Travel:
Local travel to patient residences in Dingmans Ferry and surrounding areas.
Experience
Qualifications:
Required:
Bachelor's or Master's degree in Physical Therapy from an accredited institution.
Licensed Physical Therapist in the state of Pennsylvania (PA).
At least 1 year of healthcare experience.
Proficient with Electronic Medical Record (EMR) systems, including practice-specific EMR software.
Valid driver’s license and reliable transportation for local travel.
Ability to manage time independently and meet critical charting deadlines.
Preferred:
Experience with any of the following EMR systems: Electronic Medical Record (EMR), Practice Partner EMR.
Additional certifications or experience in home health or similar settings.
